The latest version of Practical Password Reset can be downloaded from
http://www.practicaldesign.com/download/PracticalPasswordReset.zip

To upgrade/move:

1. If the web.config file had been secured, unencrypt the config file using the settings
2. Make a copy of these files
PracticalPasswordReset\web.config
PracticalPasswordReset\application.log
PracticalPasswordReset\App_Data\domainpolicy.xml

3. Install Practical Password Reset on the new server
4. Run Setup.exe from PracticalPasswordReset.zip using the default options during the install
5. Replace web.config, application.log, domainpolicy.xml with the copies you made earlier.
